This design study explores the generation and validation of chart-based visual summaries for patient history records, aimed at supporting Structured Medication Reviews (SMRs) conducted by general practitioners and pharmacists. Mock SMRs conducted by clinician co-authors provided insights into the requirements of SMRs, which informed both the design process and data abstraction. The study elaborates on this systematic process of generating a data abstraction that enabled exploring the design space using charts. Fourteen unique chart types visualising patient history records were evaluated by eight clinicians and 11 non-clinicians, with 11 of the charts deemed suitable for visualising the records. The key contributions of this work are characterising SMRs to inform data abstraction by conducting and analysing mock SMRs, exploring the design space, and reflecting on the findings.
